Home From Hospital

As soon as someone no longer needs acute hospital care, they want to be home and the evidence is that they will recover sooner at home.

Unfortunately, people often stay in hospital more days (often many more) than they need to and this longer stay weakens both their muscle strength and their morale. The delay often happens because there aren’t always arrangements to oversee their safe return home and to ensure that they have all that they need to recover safely at home in the weeks following their hospital discharge.

Who is this service for?

My Respite Care specialises in making all the arrangements necessary for safe and fast discharge for people who are medically ready for hospital discharge. We arrange:

  • For them to have a Home Discharge Plan
  • Transport home
  • Check that their home is ready to receive them, that the heating works and that there is food in the fridge
  • That any adaptations to living at home are in place or will soon be in place
  • Check that they have all the medications they need for the next few days and prescriptions for the next week before leaving hospital
  • Provide settling in care  for their first days home
  • Arrange longer term care as required
